Enrollment Process

STEP 1: Fill-up the application form HERE

Once the application form is submitted, kindly check your email for further instructions.

STEP 2: Settle the downpayment. Kindly check your email for the instructions on how to make payment.

Step 3: Submit available admission documents.

  2. Upon successful registration, the Student NumberBranch numberBranch email address, Information on payments, and Requirements for submission will be sent to your email address.
  • Upon Enrollment
    • Provide the Student LRN in the link sent to your email address
    • Submit the scanned/picture copy of the following requirements through the link sent to your email address:
      • PSA Birth Certificate (or NSO)
      • Form 138 (Report Card)
  • Pay the Admission Fee (see payment options)
  • Once payment is received, you will receive an email for the instructions to submit the following:
    • New Student Information Form
    • Student Health Information Form
    • Confirmation for:  Enrollment Agreement, Parent Social Contract, Data Privacy and Student Photo and Video Release
  • Submit the original copies, as soon as possible, the following through your respective branch or via courier:
    • PSA Birth Certificate
    • Form 138 (Report Card)
    • Form 137 (Permanent Record)
